Please use one of the forms below if you need to make a claim.
1) Property Claim form
For all claims relating to damage to your home, contents, buildings or property, including commercial buildings and commercial property.
2) Animal Claim form
If you are claiming for the theft, illness, loss or injury to an animal, other than Brucellosis and T.B. Reactor Claims (see below).
3) Liability Claim form
If someone has, or you are concerned that someone will, make a claim against you, other than arising out of a Motor Accident.
4) Vehicle Claim form
For all Motor Claims, including Motor Claims involving Fire and Theft.
5) Windscreen Claim form
For Motor Claims where the damage, to the Insured Vehicle, is restricted to 'glass' forming the windscreen or windows of the Insured Vehicle.

Personal Accident Claim Form
For all Claims made under Personal Accident and Sickness Policies including Group Personal Accident and Sickness Policies.
Brucellosis and T.B. Reactor Claim Form
For all Claims made under Brucellosis and T.B. Reactor policies.
If you cannot find the form you need, or would prefer to receive a claim form by post, please email claims@cornishmutual.co.uk, or telephone 01872 277151 and ask to speak to a Claims Handler.
To view these files, you will need to have Adobe Reader installed on your computer. Most users will find this software is already on their system, and the files will usually open without a problem.
